Printable Mail-Order Form

If you do not want to place your order online using our secure shopping cart, you can still place your order by mailing it. Just print out the Printable Mail-Order Form, fill it in and send it to us with payment.
PAYMENT:
Check/MO - We accept personal and business checks, cashiers checks and money orders. Please do not send cash or stamps. We do not accept COD deliveries. Make checks and money orders payable to: How-to-Plans.com. Payments must be in US$.
Credit Card - If paying with Credit Card via mail, complete the Credit Card Information section.
PRINTING & MAILING:
1. Click the "Print Form" button at the top of the Printable Mail-Order Form so that only the form will print. This will save ink in your printer!
2. Fill out the form making sure to include all important info (name, shipping address, item information, etc.). Please print - if we can't read your handwriting your order will be delayed. It is also a good idea to include a phone number so that we can contact you if there is a problem.
3. Make sure to add the correct shipping and handling fees. For customers in the USA see Ordering Info for the s/h fees table. Customers outside the USA must contact us for the correct amount.
4. Send the completed form along with payment to: How-to-Plans.com, 2169 Grand Blvd., Belle Vernon, PA 15012.
PURCHASE ORDERS:
We accept purchase orders from corporations, government, schools and other institutions. It is best to Contact Us first before placing your first order.
